Initial doctor or hospital visit, X-Ray or MRI reports, narrative reports including surgical reports that detail your injuries/treatment. We will work with your attorney to get the required documentation.

 

3. Defendant(s) Insurance Information:
Including contact information, claim or policy number and policy limits. We do not contact the defendant or defendant's carrier.

 

Medical Malpractice/ Product Liability

1. Expert Report
